How APCM works in aflow

Advanced Primary Care Management (APCM) is a monthly care management program. aflow helps your practice find patients who may be eligible, document their consent, prepare and review care plans, and track each month's reviews. Here's how the pieces fit together.
Where you work
- The aflow side panel. Open it next to your EHR. When a patient's chart is open, their APCM card shows their APCM status and links to their care plan. The APCM button at the top of the panel opens the APCM worklist and shows a count of items waiting for you.
- The APCM dashboard in aflowOS. Select APCM in the aflowOS sidebar. The Overview tab shows active patients, monthly review progress, and eligible patients who aren't enrolled. The Patients tab lists patients by status. Select a patient to see their APCM audit log. Users with billing access also see a Billing tab that lists each month's reviewed patients as a superbill you can export to CSV.
How a patient moves through APCM
- Eligibility. aflow marks a patient as eligible when the primary insurance on file is traditional Medicare. Other patients show Not eligible for APCM with the reason.
- Enrollment and consent. A team member reviews a short verbal consent script with the patient and enrolls them from the side panel. aflow records who documented consent and when.
- Initial care plan. After enrollment, aflow drafts a care plan from the patient's chart, usually within an hour. A clinician reviews it, edits it, and accepts it.
- Monthly reviews. Each month after the plan is accepted, aflow opens a 30-day review. If new outside results arrive, such as labs, consult notes, imaging, or a hospital discharge summary, aflow may open an Escalation review sooner.
Keeping track each month
- Monthly reviews are due by the end of the month. Escalation reviews are due two days after they open.
- The APCM card on the side panel's home screen shows how many reviews are pending and how many patients have been reviewed this month. The dashboard's Monthly reviews card shows the same progress.
- aflow shows an estimated APCM level for each enrolled patient. If it can't confirm the patient's Medicare details, it shows Eligibility unverified instead of a level.
- If your practice is set up for Medicare eligibility checks, aflow runs them automatically. The patient's card shows a Medicare row when something needs attention, such as a missing Medicare ID.
aflow shows what it knows about eligibility, levels, and completed reviews, but it doesn't make billing decisions. Confirm program and billing requirements with your billing team.
Getting access
APCM is turned on for each user, and billing access is turned on separately. If you don't see APCM in the side panel or in aflowOS, contact support@aflowhealth.com.
