Enroll a patient in APCM

Enroll an eligible patient in Advanced Primary Care Management (APCM) from the aflow side panel and document their verbal consent. Enrolling also starts the patient's initial care plan.
Before you start
- APCM must be turned on for your aflow account.
- Plan to review the consent points with the patient before you enroll them.
Enroll from the patient's chart
- Open the patient's chart in your EHR, then open the aflow side panel.
- On the patient's APCM card, select Enroll.
- Review each verbal consent point in the dialog with the patient:
- APCM is a program that helps us keep track of your health between visits
- You can stop anytime
- Only one practitioner can bill APCM each month
- Medicare cost sharing / copay may apply
- If the dialog shows No visit found in the last 3 years, the consent checkbox also confirms that the patient was seen in your practice within the last 3 years. Check it only if that's true.
- When the patient agrees, check Verbal consent obtained.
- Leave Set up patient portal access unchecked unless your practice uses the aflow patient portal.
- Select Enroll.
What happens next
- The card shows Enrolled, and the care plan shows as being prepared. aflow drafts the initial care plan from the patient's chart, usually within an hour. When it's ready, the card shows Initial care plan with a Review button.
- If the card shows a Medicare row, such as No Medicare ID on file, use the button on that row (for example, Add ID) to fix it.
- The enrollment, who documented consent, and when are saved to the patient's audit log. To see it, open the APCM dashboard in aflowOS, go to Patients, and select the patient. Select Enrolled in APCM to see the consent points.
Other situations
- The chart isn't open. Select APCM at the top of the side panel, open Patients, filter to Eligible, and select the patient to open their APCM card.
- The patient declines. Select Decline, then Decline enrollment. The patient stays eligible, and you can select Enroll on their card later.
- The card says Not eligible for APCM. aflow looks for traditional Medicare as the primary insurance on file. If you've confirmed that the patient's plan covers APCM, such as some Medicare Advantage plans, open the menu on the card, select Override and enroll…, and complete the same consent steps. Check with your billing team first.
- The patient wants to stop. On the enrolled card, open the menu and select Unenroll from APCM. Choose a reason if you like, then select Unenroll. This ends care management and clears open tasks. The care plan stays on file.
